Objetive
To discuss essential rights-based interventions to improve comprehensive health services for transgender women and to recognize good experiences in the region and the challenges to improve health responses for transgender populations in the framework of the Human Rights and HIV Response Day.
Introduction:
(Since the beginning of the HIV epidemic, a great effort has been made in research, which allowed that today, numerous tools are available that improve the quality of life of people with HIV or who are at risk of acquiring it. Currently, there are reliable diagnostic methods and treatments that can be administered by different routes of administration, with good tolerance and low rate of adverse events, and that can be used not only for the treatment of an established infection but also as a method of prevention in people who require it. Some populations are disproportionately affected by HIV and can benefit from specific prevention strategies. This would contribute to achieving the proposed 2030 targets. Without clear progress on human rights, these advances are limited.
Trans women bear a disproportionate burden of stigma, discrimination, and social determinants of health, leading to reduced access to health services. This is particularly true for trans migrant women, both sex workers and non-sex workers, and informal trans workers. These individuals often struggle to access ARVs when in transit or in countries where their documents are not recognized, creating barriers to adherence and continuity of ARVs, PREP/PEP, and other services. The health needs of the transgender population remain largely unmet, presenting significant challenges.
Given these challenges, it is imperative that we address them from the standpoint of human rights for transgender women and the perspective of comprehensive health care. This should be a top priority for governments, national HIV and STI programs, organizations, and international cooperation and strategic allies. Only by adopting this approach can we hope to meet the health needs of the transgender population.
On the International Day of Transgender Memory, World AIDS Day, and International Human Rights Day, we, the Horizontal Technical Cooperation Group, UNAIDS, and the Pan American Health Organization, along with the support of regional networks and community-based organizations, are providing a platform for dialogue. This dialogue is aimed at addressing the need for comprehensive health services for transgender women. We aim to recognize the positive experiences in the region and the challenges that need to be overcome to improve health responses for transgender populations.
